Cancellation Policy

Payment is due at the time of appointment unless received prior to appointment. You may cancel your in-office test with a full refund within 24 hours of the scheduled appointment. To cancel and reschedule, please contact us at 743-240-4767 or at info@phenomlabs.org. Office appointments cancelled less than 24 hours prior to the scheduled time may incur an administrative fee equal to 50% of the cost of test. Refunds cannot be processed after you visit the office and provide blood or other specimens to us for testing. However, if Phenom Labs cannot process your specimen you will receive 100% refund.
