According to the HIPAA privacy rule (45 CFR Part 160), Phenom Labs may not disclose or otherwise use protected health information without consent. You have the right to examine and request a copy of your health records. As a patient, you also have the right to direct Phenom Labs to send a third-party electronic copy of your protected health information in an electronic health record and to request corrections to the record be made. 

HIPPA Security Rule (45 CFR Part 160) requires Phenom Labs to implement certain standards to protect any electronic protected health information that we create, receive, use, or maintain. Phenom Labs takes appropriate administrative, physical, and technical safeguards to ensure the confidentiality, integrity, and security of protected health information.

The Breach of Notification rule (45 CFR §§ 164.400-414) requires Phenom Labs and our business associates to notify you following the breach of unsecured health information. 

If you believe Phenom Labs or our business associate has violated your (or another person’s) health information privacy rights or committed another violation of the Privacy, Security, or Breach Notification Rules, you may file a complaint with the Health & Human Service’s Office of Civil Rights (OCR) who has the ability to investigate.
